Medical Quality Improvement Program
The Medical Quality Improvement Program (MQI Program) is designed to improve the quality of care provided to a healthcare organization’s patients, while improving the process of collecting the patient health data. Through the use of Telemedicine and house dispatched nurses, visiting physicians can make real-time “house calls” to provide on-demand medical services and provide a comprehensive method of data collection.
The “house call” visits do not replace the patient’s primary doctor and PCP visits. Instead, the program compliments existing care provided to the patient. Ideal patients selected for the program include patients with recent hospital visits, frequent readmissions, frequent visits to the ER, and patients with multiple illnesses. Telemedicine can be very useful to patients who are physically impaired or live in remote areas. However, all patients will benefit from scheduled visits twice per year.

Improved Quality of Care
- The Delta Medical Care, Inc. medical staff performs a complete medical assessment. As part of the assessment, a member survey will be completed, which can be designed based on the needs of the provider or health organization.
- The results from the visit are documented within the patient’s medical records; certified by the consulting physician and provided to the patient’s primary care physician.
- In some cases, the staff will send additional communication to the primary care physicians to encourage scheduling for necessary tests or procedures based on the evaluation performed.
Improved Quality of Documentation
- Complete and accurate documentation of the patient’s health conditions and diagnosis will be performed for each encounter.
- Data files with the ICD9 diagnosis codes from the encounters are prepared to assist the client in data analysis and for submissions to applicable regulatory agencies.
- Medicare health plan patient’s data collection process will be focused on both ICD9 data for risk adjustment submission and key information impacting health plan HEDIS measures.
The MQI Program will improve the quality of care provided to patients. Data submissions to regulatory agencies will be improved to accurately reflect the current conditions of health plan members and their provided care. The result of accurate and complete data submitted to CMS will optimize revenue earned for Medicare health plans.
